What is it?
Our Brief Service model is a short-term, goal focused approach, which helps people address their concerns by exploring their strengths, abilities and navigating their use of resources. Our team’s approach utilizes 1 to 3 sessions.
Goals

Generate collaborative and meaningful goals.

Co-create a plan to achieve identified goals.
Who is it for?
- Children, youth (up to the age of 18), families and their supports.

Brief Services:

Solution –focused sessions (in person, by phone or virtual)

Case Management and Service Coordination: consultation with other professionals and agencies who are also involved with your child and family; referrals to other services in the community

Referral to Tele-Mental Health (access to child psychiatric consultations by video conference)

Linkage to the Huron Perth Helpline and Crisis Response Team for situations for which crisis intervention may be needed
